House Call Program

You may ask for a house call through your HealthWise provider.  The House Call Program is a collaborative effort between HealthWise and the Visiting Nurse & Health Services of CT and utilizes the services of Geriann Gallagher, APRN, to provide house calls to our patients.  Please contact your HealthWise primary care provider for more information.
